The Laughing Dog Inn, Inc. agrees to exercise reasonable care and to keep its kennel facilities sanitary and properly enclosed. The dog is to be fed properly and regularly and to be housed in clean and safe quarters. Owner(s) or persons boarding the dog assume all risk for loss or damage from disease, death, running away, theft, fire, injury to persons, other animals, or property or owner unavoidable causes, due diligence and care having been exercised.

If the dog becomes seriously ill the owner shall be notified at once and the Inn’s local veterinarian shall be called. In cases of dire emergency the dog will be taken to Thousand Oaks Emergency Hospital. The owner is responsible for all veterinary charges incurred and will fully compensate the Inn for any money paid to the veterinarian in charge.

All charges incurred at Laughing Dog Inn are to be paid at the time of pick-up by check, cash or credit card. If the dog is not picked up within 21 days after time for return of the dog, the dog will be placed in a home or a humane society. Notice in writing of such intended disposition shall be mailed by Certified Mail to the owner of the dog at the address given, not less than 10 days before date of such intended disposition. All charges as well as excess amounts over charges will be given to the owner of the dog. Any deficiency and legal or collection fees are to be paid by the owner.

No minors under the age of 16 are allowed on the kennel premises. The parent(s) of the minor, age 16-18, assumes full responsibility for any injuries sustained by the minor on the premises.

The person submitting this form represents that he/she is the legal owner or agent of said dog and that said dog has not been exposed to contagious diseases in the last 30 days and that the required inoculations have been obtained.
